Client Forms for Blue Heron Healing

If you have scheduled an energy healing session, please complete the forms below and return them to me prior to your appointment, if possible, or you may bring them with you. Download the forms and mail them to me at 664 Longmeadow Street, Longmeadow, MA 01106  or email them to me at Julie.Montinieri@gmail.com.

Please also take a moment to review my policies.  You may send the policy form with your signature or I can provide you with one at our first meeting.(Ignore this for now as I can't seem to post my policies!) I'll provide you with a copy at your appointment.

My fee is $149 per session as most run about two hours.

This information will enable me to prepare to meet your healing needs in a comprehensive way.
